Nuisance Complaints

To file a nuisance complaint, complete the form and mail or e-mail the completed form to the Health Department.  A completed form must be signed before the complaint can be investigated.  For persons not willing to sign the complaint contact one of your local Township Trustees and ask him/her to sign the complaint.
